* Allow mixing text styles, e.g. _*bold and underlined*_
* '+' has been changed to a span without style change
* '+++' for passthrough
* ^ and ~ for superscript and subscript
* trailing ^ in link text means target = _blank
* simplify phrase modifier wrt word boundary checks
