Right click on the SBlock element shown in "Model Explorer" and select "ESF Analysis Tools > ESF FMEA > Start a new FMEA".
In the SSafetyArtifacts element, three nested elements are created:
Double click on a FMEATable to open it.
List of the fields of the FMEA Table:

| Causes | Identification of one or more causes that trigger a process (mechanism) that leads to a failure mode. |
| Local Effects | List of the failure effects applied to the item (SBlock) under analysis. |
| System Effects | List of the failure effects at the global system level. |
| Customer Effects | Level of severity for the worst case unfavourable to the end (system) effect. This level is defined at the beginning of analysis. |
| Initial Severity | Level of severity for the worst case unfavourable to the end (system) effect. This level is defined at the beginning of analysis. |
| Initial Occurrence | Estimation of the probability of the frequency of the failure mode. This estimation is defined at the beginning of analysis. |
| Is Detectable | Define if there is at least one way to detect the failure mode. |
| Initial Detectability | Estimation of the means or the method by which a failure is detected. This estimation is defined at the beginning of analysis. |
| Initial Criticality | Measure used for risk assessment of failure modes. Its value is automatically calculated: Initial Severity x Initial Occurrence or, if it is detectable, Initial Severity x Initial Occurrence x Final Detectability. This estimation is defined at the beginning of analysis. |
| Initial Preventive Actions | Additional information in relation to the measures used to reduce the risk or to justify a level of risk or the proposed scenario. This information is defined at the beginning of analysis. |
| Final Severity | Level of severity for the worst case unfavourable to the end (system) effect. This level is updated if necessary with each analysis. |
| Final Occurrence | Estimation of the probability of the frequency of the failure mode. This level is updated if necessary with each analysis. This estimation is defined at the beginning of analysis. |
| Final Detectability | Estimation of the means or the method by which a failure is detected. This level is updated if necessary with each analysis. |

For add a new failure mode there is two ways:
First way it is directly on a FMEA Table. Right click and select "ESF Analysis Tools > ESF FMEA > Create a new Failure Mode".
Second way it is directly on a SBlock shown in "Model Explorer". Right click and select "ESF Analysis Tools > ESF FMEA > Create a new Failure Mode".
There is a synchronization between the model and the FMEA Tables. The result of actions of "add" and "remove" a failure mode is reflected on both sides.
The field "Initial/Final Criticality" is automatically calculated: Initial/Final Severity x Initial/Final Occurrence.
If the field "Is Detectable" is marked, the value of "Initial/Final Detectability" is considered in the calculate.
