Bugs 48869 and 50244. This implements a scheme of replacing tool bar contribution items with placeholders when they are disposed by EditorActionBars. Still to investigate: verify the memory leak is still gone (should be); verify Nick's patch doesn't display override craziness; and verify that ActionSetActionBars don't need placeholders as well.
