Bug 546219: [Commonmark] Rework source blocks
- Rename *BlockItem to *BlockNode in accordance to common AST
terminology and for better distinction of list item
- Rename SourceBlock to SourceBlockType to reflect its function
- Improve SourceBlockBuilder to provide access to more information,
especially when parsing nested nodes
