Bug 571892 - [GTK] Clean up old/unused functions in ExpandBar and
ExpandItem

- Removed ExpandItem unused fields, x, y
- Removed private wrapper function ExpandItem.clientHandle() to return
clientHandle field
- Removed private function ExpandItem.getHeightInPixel which was used
only in getHeight
- Removed usused index parameter in ExpandBar.layoutItems function
